Please print the submission sheet and fill out all fields and submit it with your order!

For full shipping instructions, please refer to the Terms and Conditions page. Place cards and the submission form inside a box or bubble mailer, and direct it to the shipping address provided in the top left corner of the submission form.

*You must print the submission form and fill it out accordingly with your order! Make sure you sign and date the sheet.

If you have trouble printing the submission form, take a blank piece of paper and list each card (Player/Character, Year, Set) at the top or bottom write the following

"I, (Your Name) hereby agree to the terms and conditions of my Black Gold Grading submission (Order #)" and below this please sign and date to make sure it's a painless submission*
